Please print liability form, complete and notarize, scan and email back as attachment to LeaderApp@iccinc.org with LIABILITY WAIVER-STUDENTS NAME in the Subject line (ex. LIABILITY WAIVER-SHERRI FREEMAN). Completed form will be valid for a period of one year. There is no need to complete a waiver for any additional Student Leader applications within that year.
